Geriatric Services for Ages 65 and Up
Community Support Program (CSP)
Short-term services for adults and children within the community setting includes navigating systems related to follow-up regular health maintenance, general assistance with day-to-day activities or helping to reduce problems (medical and behavioral) that may related to health concerns, helping with transportation to these appointments. Establish and maintain linkages necessary for support ability to live successfully in the communities, jobs, education or other resources.
Driver Alcohol Education (DAE)
Our Driver Alcohol Education (DAE) programs are available to those individuals who need the Bureau of Substance Abuse Services Approved Program. Specifically, each DAE program participant a structured group where they receive Dialectical Behavior Therapy blended educational material to help them identify and understand alcohol abuse issues and drinking-and-driving behaviors. While the major focus of these programs is on alcohol, other substances of abuse are also discussed. The program provides 40 hours of services conducted over 16 weeks and includes an assessment, participation in self-help and victim-impact community meetings.
Eligibility: Individuals convicted for the first time for drunk driving and who choose this option as an alternative to losing their license or possible incarceration. Referrals are made by the district court.
Recovery Coach (RC)
Our RC program provides daily assistance with activities to support the voices and choices of the PATI who are on the recovery path in the community by minimizing the power differentials as much as possible. The focus this program is to create a relationship with the individuals within the community who are non-clinical and are focused on removing obstacles to recovery; linking members to the recovery community and serving as a personal guide and mentor. Our Recovery Coach will work with the patients to develop a Wellness Plan that orients towards the activities of the RC service and support them in obtaining and marinating recovery.
Eligibility: Patients can access RC services based on medical necessity and a referral by a medical or behavioral health provider, Community Partner (CP), or other care manager who has contact with the Patient and is able to identify the need for RC services.
Recovery Support Navigator (RSN)
Our RSN program provides assistance, care management, and system navigation support to patients with a diagnosis of substance use disorder and/or co-occurring mental health disorders. Our RSN Program explores treatment recovery options with the patient, which helps to clarify goals and strategies, provide education and resources, and assist patients in accessing treatment and community supports; and/or develop and implement personal goals and objectives around treatment and recovery from addiction and/or co-occurring disorders.
Eligibility: Patients can access RSN services based on medical necessity and/or a referral by a medical or behavioral health provider, Community Partner (CP), or other care manager, that has contact with the Patient and is able to identify the need for RSN services.
